Emergency Services - Qualifications Renewal
ES qualification renewals must follow the
Texas wing Supplement to the ES regulation, CAPR 60-3.
The correct procedure is as follows:

Go to e-Services,
▬► My Operations Qualifications/National Reports
▬► Emergency Services
▬► SQTR Worksheet, then
▬► Pick Achievement and select the one you want from the
scroll-down list

Do not touch your existing online qualifications.
The information on this page can only be changed by
Texas Wing HQ. If you type in any changes yourself,
fixing what you did could take a very long time.

Print a copy of your Specialty Qualification Training
Worksheet for that achievement.

Have a member who has assessed your knowledge and
is SET-qualified sign you off by writing, on your SQTR
Worksheet, "Requalified for [specify the rating]" then
the SET-qualified Member's Signature, CAPID, date, and
mission number. For example, Capt John J. Johnson, CAPID
999999, would certify your re-qualification as follows:
Requalified for Mission Radio Operator, John J.
Johnson, 999999, 1/21/08, 08-T-9999

For best results, scan the signed sheet, save it as a
PDF file, and attach
the PDF to an e-mail addressed to the Group III ESO
pperkins@iswebhosting.com - On the SUBJECT line,
write "SQTR Requalification."
▬ If you cannot create a PDF file, fax the document to
Major Perkins at 214-256-4520

The Group III ESO will keep a copy for your records,
then send your re-qualification to Texas Wing for
posting.

Texas Wing will then input the renewal for approval.

Pilots - please include a copy of your
FORM91 with the SQTR documentation.

Any ES qualification not entered by Texas Wing will be
declined at the Group III level.

NOTE1 |
The tasks need to be approved by a trainer who
is on the
TXWG SET List FOR THAT SPECIALTY. Always
check the SET List posted on the TXWG website.
Do not use a list you downloaded and saved to
your computer, since it could be out of date. If
the trainer is not on the online list, the
requalification will be invalid. |

NOTE2 |
The group commander will not approve mission
base staffing tasks that are not performed at an
ICP. This includes Branch Directors, Section
Chiefs, CUL and above. These tasks cannot be
properly trained at a staging area. Please
direct any questions to him on this policy. |
